Here is a link to the ATSB final report on QF32 released today. Many details were sort of known of, but the extensive report outlines how big the damage to OQA was. And how Rolls-Royce failed to identify for some time that their manufacturing process of making oil feed pipes to the required standards was wrong and could have had such dramatic results.
