There are certain areas assigned over which aircraft to do that. I think there is one in the middle of nowhere over Northern Germany. Found this video of an A330/A340 doing it. Title is wrong, but what it's actually doing is dumping fuel. Found this photo from Farnborough, where you can see the hose in detail on A380 MSN 1:

Title is wrong, but what it's actually doing is dumping fuel. As long as the altitude is sufficient the fuel will anyway evaporate before it hit the ground
