Then clearly A320 production is already running faster than 42 per month. Airbus made plans long ago to raise production to 46 narrowbodies per month (= 138 per quarter).

This might be an odd question but I'm curious to know how are productions measured? Per Frame assembly, First Flights or Deliveries per month?

Airbus made plans long ago to raise production to 46 narrowbodies per month (= 138 per quarter). But SPIRIT is already half a year running at over 48/month - so we might conclude that the current rate is 48 ! Sounds like the planned ramp-up towards 50 per month in 2017.

We can guess that Airbus probably has kept the -ceo production rate unchanged (or almost unchanged) to meet strong demand, while -neo assembly started. This amounts to a rate increase, so far hidden by -neo delivery delays.
